I am making a hard lemonade - it's simply lemonade concentrate, honey, turbinado sugar and wyeast 4021. I made it on 4/7 and on 4/9 saw no activity. I took a hydrometer reading and there was no change. On the evening of 4/9 I added another pack of 4021 wyeast, yeast nutrient and diammonium phosphate expecting to see it going crazy this morning.
However, it still hasn't bubbled and the hydrometer reading hasn't changed. Any suggestions?

The first thing I would suspect is the lemonade concentrate. If it contained preservatives, it may never ferment. Look for sorbates, sulfites etc on the label.
Next, I would suspect the yeast. The pouch was properly activated and puffed up?
Lastly, I would wait longer. It has really only been 3 days. If after 2-3 more days it isn't fermenting, then I would put in some dry wine yeast.
